Complete a Request for Copy of Tax Return (Form DR-841) and email it to recordsrequests@floridarevenue.com, mail it to the address on the form, or fax it to (850) 922-5936 or (850) 922-0861.
If you filed the return online, visit the Department's eFile and Pay Taxes, Fees, and Remittances webpage and log in with your user ID and password. Scroll down to the bottom of the page under "Other Options" and click "Submitted Confirmation Report." You can select the confirmation page you wish to reprint by clicking on the confirmation number. Online history is only available for the past six months.
